Transverse process midsegment erector spinae block technique in lumbar surgery and lumbar transverse process syndrome

Abstract:The erector spinae plane (ESP) block is a well-established ultrasound-guided nerve and fascial plane block technique.By targeting the course of the nerves, local anesthetics are injected at either the tip or the midportion of the transverseprocess to block both the dorsal and ventral rami of the spinal nerves, thereby producing an antinociceptive effect. In this study,a novel needle insertion approach was adopted: with the patient in the lateral decubitus position, injection was performed at themid-transverse process under“ shamrock” imaging guidance. This technique achieved improved analgesic outcomes in both spinalsurgery analgesia and the treatment of third lumbar vertebral transverse process syndrome.
